Application for services related to correction/modification/ change in the name of a member to be submitted through the
"Change of Name" module within the login page. No fees applicable. The following documents are mandatory to process the application.
- Copy of the gazette notification announcing the change in name
- Copy of the newspaper which published the change
- Original of affidavit duly executed by a notory
- Copy of marriage certificate (if name effected as a result of marriage, applicable to female candidates)
